i haven't been able to fully reproduce this.  some things i've
determined:

- using the latest dbconfig package and your otrs2 package, the
  installation succeeds on my system.  
- it *does* ask for the dbuser password as part of the debconf
  configuration.
- the database gets populated, though it uses password and not ident
  as you noticed.
- i wasn't prompted for a password during this populate step[1].
- later, i can connect as the otrs system user via ident without a
  problem, and access the schema.

[1] what version of postgresql are you running?  i did this testing on
    8.1.  i know that 8.0 has insurmountable problems with
    noninteractively supplying passwords, but it's no longer part of
    debian (only 7.4, which i think works too, and 8.1).
