Added changelog entries up to r6783; entries will be moved to separate file in next commit

The complete (developer's) changelog will reside in doc/dev-changelog. All
entries currently in debian/changelog for 4.0* will be moved to that file. This
intermediate commit is merely to ensure consistency and completeness.
