[newmaint-site] Bug#795994: nm.debian.org: implement stating the intention to start a process

Hello,

currently the site has lost the NM step of stating one's intention to
become a DD, and a new process is created when the advocate clicks on an
advocation link. This has a few issues:

 - advocates click on the wrong advocation links
 - applicants might get advocated nonconsensually
 - the intention to become a DD does not get posted anymore to
   debian-newmaint.

I would like to reintroduce the possibility of stating the intention to
apply for a new status in Debian. Here is how I propose it:

 - in my personal page, maybe next to the "current status in the
   project" field, add a link "request a change"
 - in a new view, I get to choose what I want to become among the
   various valid options, and to enter some free text stating my
   intention
 - once submitted, a mail with the text is sent to debian-newmaint
 - a new process is created, waiting for advocacies
 - the right "advocate" button appears on the person page for advocates
 - a mail is sent to the applicant with explanation of what to tell the
   advocate to do, and with things like "for questions, feedback, or if
   you want a reality check if things do not go as you expect, you can
   contact nm at debian.org or your advocate(s) at any time"
