FWIW, the way I was able to get the desired result was with texlive
and pdfpages, manually adding empty pages where needed:
\documentclass{book}
\usepackage{pdfpages}
\begin{document}
\includepdf[pages=-]{01:foo}
\includepdf[pages={1-5,{}}]{02:bar}
\includepdf[pages={1-3,{}}]{03:baz}
[...]
\end{document}
