This looks to me as if it was either of the following two problems
(where the first one is - by far - the more likely one):
1) An already existing BerkeleyDB 4.* (or other) file in
   /var/lib/cyrus-postillion/mailboxes.db
2) A problem with permissions
At least I was perfectly able to convert a flat mailbox list file from
my cyrus-imapd-2.2 sarge installation to a db3 or db3-nosync mailbox
file in a sid cyrus21-imapd temporary installation.
