Hi there,

Well I just did a fresh install of evolution and evolution-exchange on a
freshly installed machine.
The version of evolution-exchange is 2.12.2.dfsg-1+b1

And, again, all I get when I choose "Microsoft Exchange" is the Username
field. The Forward button is greyed, even after typing an Username.

Very puzzling.
Did you try creating a new user, login as that user, than start up evolution
?
There might be some "leftover files"

Cheers,
