I looked at our (SableVM) import of GNU Classpath for inspiration, which
also uses GPL with some kind of an exception. See:

In this case COPYING contains pure GPL. Please also note that the
beginning of the GPL says:
"Everyone is permitted to copy and distribute verbatim copies
of this license document, but changing it is not allowed."
So, putting the exception in this document doesn't seem allowed.
Instead in case of Classpath they have all the "dirty stuff" in the
LICENSE file, which contains informations about the actual conditions
under which software is distributed.
As for the upload to mentors - that was definitely good idea ;-)
As for "including email w/ permission from upstream" - yes, this is
a common practice, when the version you want/need to package didn't
contain the proper licensing terms.
