Patch for upgrade to gst-0.9.2

 I still did not finish reviewing your patch.  For most changes, I'll
 have to compare a rebuild of the newer tarball with the file moves you
 propose, and I'll try to do that this WE (0.9 is low priority).

 For one change though I'm surprized: the doc build patch is a patch to
 comment out a part of the documentation because openjade has a bug
 where it will loop forever if this is present.  You state that this is
 "now in upstream", what did you mean with that?
