Are you using this config (that of multipath iSCSI Paths for the Root LUN + DM 
Multipath) along with the proposed patch ?

Even the multipathd daemon will not be available in initramfs. The multipathd 
daemon does health status check for the individual paths and updates the map 
but I'm not sure if it plays any role in cycling the I/O across the paths. Or 
is it just the dm-mulipath.ko module ?
Failures for individual paths are sensed in the SCSI layer but those paths are 
declared failed only when the path checker checks it. That runs in userspace, 
Right ?
