I didn't find any official source, but both spellings seem to exist.
Liberia doesn't appear in the ISO newsletters and wikipedia has
"Rivercess" in it's article for ISO 3166-2:LR [1].

According to statoids [2] "River Gee" and "Gbarpolu" were created in
2000 and 2001, after the publication of ISO 3166-2. So, since the
entry of Liberia has not been updated, they do not seem to be included
in the standard yet (see also comment in wikipedia [3]). It would be
nice if they include Liberia in the next newsletter, we could thous see
which spelling they prefer for "River Cess".

I commited the corrections for the two other errors you reported, they
are probably typos, thank you for reporting them :)
