[pkg-lighttpd] Bug#697304: lighttpd: mod_extforward missing configuration file

Dear Maintainer,

lighttpd misses a module file in which mod_extforward is loadable (the
module itself is shipped in the package, though).
Should look like /etc/lighttpd/conf-available/15-extforward (15, because it
must be loaded AFTER accesslog!!):
server.modules += ( "mod_extforward" )
