Bug#637104: lvm2: lvrename fails to rename COW partition when renaming snapshot

lvrename fails to rename the COW partition associated with a snapshot:

  device-mapper: rename ioctl failed: Device or resource busy

Then if a rename is attempted to the name that should have previously been renamed away
(but returned EBUSY instead as above)

Further operations on the snapshot produce a hang in uninterruptible I/O state
which requires a reboot.
