This looks the same as my report this morning: #726296

The main cause for it seems to be the hunk which get lost in the
/lib/udev/rules.d/60-kpartx.rules:

# This is a temporary hack until Debian's dmsetup properly supports "dmsetup

IMPORT{program}="dmsetup_env %M %m"
