Bug#780319: initramfs-tools hook doesn't support devices named as /dev/VG/LV

The initramfs-tools hook can set up a device specified as
/dev/mapper/VG-LV but not /dev/VG/LV.  This is a longstanding
limitation for the root filesystem but now affects the /usr
filesystem as well.
