There are a few nasty issues concerning association (or lack of) reported to
the madwifi.org trac system [1,2]. The fact that you reported the use of
NetworkManager suggests to me that this may be related to the introduction of
the WEXT ioctl's for use with wpa_supplicant (this changed slightly around
r1527 or so).
When NetwrokManager is active, isn't every interface under its control "bound"
by a wpa_supplicant process? Therefore running dhclient manually while
NetworkManager subsystem is still active may never succeed.
Just my 2c, I hope these showstopping issues get ironed out of madwifi very
soon . . .
