Nothing unusual - mkfs.xfs or mkfs.reiserfs, default settings.
Anyway, according to /proc/mdstat, the block count is too low to be 5TB.

Why not? I've got similar server (runned a year ago) with raid6, 6 SATA driver 1TB each, using xfs filesystem.
