Do you have multipaths?

Why are you not using mdadm for multipaths?

Do you have the dmraid package installed?

Can you please check out http://wiki.debian.org/InitramfsDebug at
the bottom and make sure to submit the trace log of a failed boot
with the debug kernel option?
