I just tried with Iceweasel 3.0.6-3 in Lenny and the above addon, on
x86-64 and it works fine with all the xforms samples from
http://www.mozilla.org/projects/xforms/samples.html

Could you try to disable all your other extensions and plugins (using
the MOZILLA_DISABLE_PLUGINS environment variable, please see
/usr/share/bug/iceweasel/presubj)
