[php-maint] Bug#332632: pear.mk integration

Hi Uwe,

I finally got around to testing the integration of pear.mk into
dh-make-php. It appears to work beautifully. :-) I checked in a few
final perfecting changes, and now would like to see this become publicly
available.

As you can see from bug #332632, I first proposed that pear.mk be
included in CDBS 222 days ago. Per their suggestion I then pursued the
option of getting it into php-pear instead, but the maintainers have not
yet responded to that proposal.

At this point I think the best thing to do is to simply include pear.mk
in dh-make-php so that we can make it publicly available. If in the
future we ever move it into php-pear, it will simply change the
build dependencies of the effected packages from dh-make-php to
php-pear, a minor inconvenience at worst.

If anyone has an alternate proposal, please speak up. If not, we can
reassign 332632 to dh-make-php and close it with this upload.
