Aren't you missing the "Provides" field?
And for an automatic upgrade from kernel-patch-skas you're missing the
whole kernel-patch-skas package that depends on linux-patch-skas (plus
versioned Conflicts/Replaces).
Last but not least, why not creating a source package with the new
name instead?
Oh, and just for the record, there is a couple of bugs coming from NMUs
that need to be ack-ed.
