Are you running vim as 'vi' or as 'vim'? I guess the former.  In this
case the behaviour you are reporting is the expected one, since
'-- INSERT --' is shown only when vim is not incompatible mode. See the
latest NEWS item in /usr/share/doc/vim/NEWS.Debian.gz
