on some systems (eg a lenovo thinkpad edge), more than one alsa device
with the same name show up. as devices are stored as names in
/xfce4-mixer/active-card, xfce4-volumed can not be configured to use any
of them but the third.
in this very setup, the sound devices and their controls look like this
in xfce4-mixer:
* HD-Audio Generic (Alsa mixer)

* Speaker
* ...
* ThinkPad Console Audio Control (Alsa mixer)

the main volume that's supposed to be regulated by the hardware buttons
is the one marked with [2], but instead, the only thing that works with
hardware buttons is toggling [1] with the mute button.
/xfce4-mixer/active-card is configured to HDAudioGenericAlsamixer, which
fits both devices.
this, by the way, also affects /xfce4-mixer/sound-card and
/xfce4-mixer/sound-cards. those only constitute minor annoyances,
though.
in this case, a workaround could be found by installing
gstreamer0.10-pulseaudio, which names devices differently.
