changelog: use canonical "closes: Bug#nnnnn" form

Use the canonical form of "closes:" as recommended by the Debian Policy Manula,
section 4.4, and no other. Some broken changelog viewers will only link to bugs
if they include the "#", and if we're being picky about that it's better still
to use the canonical form in full.
