I had in the past, in  this case I had an old broken initramfs-tools image 
that included all the three tools, mdadm, lvm and evms, so the mess is coming 
from here.

I retried with a recent initramfs and kernel 2.6.14, now my /dev/mapper is 
empty but /dev/mapper/control.

The new results are so strange...
$ sudo /home/marco/local/sbin/yaird -v -o /tmp/thomas
.....
yaird error: Don't know how to choose between /dev/evms/sdb9 
and /dev/evms/.nodes/sdb9 for dm-28 (fatal)

$ sudo /home/marco/local/sbin/yaird -t
...
yaird error: Don't know how to choose between /dev/evms/lvm2/sicuro/games 
and /dev/evms/.nodes/lvm2/sicuro/games for dm-13 (fatal)

Now /dev/mapper should be out of question...
And I purged mdadm, dmsetup, lvm2, lvm10 in the doubt of ancient 
installations.

I also use udev, btw, If it could helps.
