Are other DVD-RW or DVD-R among the successful media ?

Do you get the same error messages from the LG ?
(fgrep for "Sense Code: 0x04 Qual 0x08" in the message output.)

It would be interesting to see the output of
  dvd+rw-mediainfo /dev/sr0
with the media already loaded in the drive. (Else the output
would be very short.)

I am not aware that the DVD-R[W] specs would have changed during
the last years. Any problem cause must be in drive or media.
