Debian policy doesn't seem to say anything about what to patch in which way.

Why? In my experience, it is a lot easier to have one patch per subject
(what the patch wants to achieve rather than what file it patches) then
it is to have one patch per file. It is also more in line with how
version control systems (well, anything more usable than rcs or CVS)
handle commits: You commit changes related to each other in one commit,
not one commit per file.
