Hi, 

during a test with piuparts I noticed your package failed to install due to 
insserv rejecting the script header. Some notes are

From the attached log (scroll to the bottom...):

  insserv: Service udev has to be enabled to start service lvm2

  dpkg: error processing lvm2 (--configure):
