it is quite easy:

see /usr/share/doc/dbconfig-common/dbconfig-common.html/ch-develguide.html

put the relevant sql commands for the upgrade to:

/usr/share/dbconfig-common/data/PACKAGE/upgrade/DBTYPE/VERSION

Nothing for is needed for the upgrade support, if the installing via dbconfig 
works already.

Have a look at the roundcube package, we also use dbconfig to update the 
database.
